Callback requests
The callback form asks only for your first name, last name, phone number, optional email address, preferred callback time, and consent to be contacted. Please do not submit symptoms, diagnoses, treatment details, or other medical information through the form.
Submitting a request does not create a clinician-patient relationship and does not confirm an appointment.
How callback information is used
The information is used only to route your callback request to the practice’s approved contact workflow. Version 1 of this website does not include a patient database or a callback-request dashboard.
The practice should confirm that any service receiving callback requests is appropriate for its workflow and that any required privacy or contractual safeguards are in place.
Security and spam protection
The callback form uses server-side validation, request limits, and Cloudflare Turnstile when configured. No internet transmission can be guaranteed to be completely secure.
